Application for Association Membership (Parent/Guardian)

I hereby apply to become a member of the Coastwide Child and Family Services Association.

I understand that my application will be tabled for approval by the Board of Management at the next  Coastwide Child and Family Services Board Meeting. In the event of my admission as a member, I agree to be bound by the rules of the Association.
